Examine how the policy can be used by the inter-professional team to ensure coordinated. For example, how well does the 30-day readmission policy work with my population? Who all is involved in the care? How does the health care team work and collaborate to prevent a 30-day readmission? Etc- so basically, how effective is the policy and is it working like the policy/legislation was intended?]
